- Assess your need for home health care.
- Answer your questions about insurance.
- Set up services, as ordered by your physician, that are covered by your insurance.
- Connect you with other community resources.
What is home health care?
Mercer Street Friends Home Health Care provides various services to help you achieve wellness and disease management. The services are delivered in the comfort of your home. They are ordered by your physician and based on your needs. They may include:
- Nursing
- Certified Home Health Aides
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Medical Social Worker
- Registered Dietitian
- Behavioral Health
Why would I need home health care?
- To help with your recovery after a hospital stay or injury.
- To provide more intensive services to help you recover when your health condition worsens.
- To teach you or your caregivers to perform injections, change dressings or other procedures.
- To provide physical, occupational or speech therapy.
- To teach you or your caregivers about an illness or condition to help keep you home and out of the hospital for as long as possible.
Who pays for home health care?
- Home health care is covered by Medicare, Medicaid and most group insurance plans.
- Most insurance plans and Medicare require that you be homebound.
- Private pay options are available.

- Talk to your physician.
- If you are a patient in a hospital or other medical facility, talk to a social worker or discharge planner.
- Most local hospitals and facilities have a Mercer Street Friends Home Health Care Nurse Liaison representative to help you with the process.
