Our nursing staff is dedicated to providing the highest quality of care to our patients and their caregivers.

Your home care nurse will provide nursing care and teach you about:

Behavioral Health Program

Mercer Street Friends Home Health Care offers in-home Behavioral Health services. Through regular visits, we help our patients maintain optimum mental health and wellness.

Our Behavioral Health Team includes a variety of medical professionals and paraprofessionals.

The Nurse:

  • Assesses psychosocial status
  • Monitors medications
  • Provides support

The Home Health Aide:

  • Reinforces coping skills
  • Promotes independence with self-care
  • Helps with meals, bathing, dressing, and other personal care
  • Helps with laundry and grocery shopping

The Rehabilitation Team provides:

  • Physical therapy
  • Occupational therapy
  • Speech therapy

The Medical Social Worker:

  • Assesses your resource needs
  • Refers you to community services
  • Teaches and reinforces decision-making

The Registered Dietitian:

  • Provides individualized nutritional assessment and education
  • Teaches and reinforces healthy eating

Chronic Care Program

The Mercer Street Friends Chronic Care Program serves people with long-term needs.

Our Chronic Care Team includes Nurses, Home Health Aides, Rehabilitation Therapists, a Registered Dietitian and a Medical Social Worker.

Our services include:

  • Mediplanner prefill
  • Injections
  • Personal care
  • Health education
  • Home health aide supervision
  • Insulin prefill
  • Catheter and tube change
  • Nursing assessment

We partner with other community agencies:

  • Mercer County Board of Social Services
  • Mercer County Office on Aging (U.S. Title XX)
  • Community Caregivers Program for the Elderly and Disabled (NJ)
  • Jersey Assistance for Community Caregiving (NJ)
  • Caregivers Assistance Program (Mercer County)
  • NJ Medicaid Personal Care Assistance
  • Mercer County Respite Care Program
  • NJ Division of Developmental Disabilities
